Financial Assistance through the Sliding Scale Program
As a service to our community, BRMC is pleased to be able to provide financial assistance to needy families through our Sliding Scale Program. Funding for this program is provided by the Health Resources and Services Administration (HRSA) of the US Dept. of Health and Human Services (HHS). As a result, we can offer a discount on medical fees to qualified individuals and families. Sliding Scale eligibility may also qualify individuals and families for special discounts in the dental center and pharmacy.

You may complete the application and return it, along with documentation of all household income(s).

Allow two weeks for processing. Sliding Scale eligibility must be in place before the day of a medical/dental appointment, or pharmacy need. We look forward to assisting you and your family!
