The Rural Health Outreach Program
The Rural Health Outreach Program (RHOP) strives to bring vital health
care to those in Nelson and Amherst Counties and surrounding communities
who most need our support—not only those of low-income but also folks
who are homebound, unable to drive, elderly, ill, handicapped, in crisis and
lonely . . . those who tend to slip through the cracks of the traditional health
care model by removing barriers that limit access to care and by finding
solutions that work both for individuals and for the community as a whole.
The Rural Health Outreach Program has served Nelson County since 1992,
making comprehensive, accessible, and affordable health care available
to residents of central Virginia regardless of their ability to pay. To access

RHOP’s comprehensive services include:
Health Depots: Community clinics at local sites offer basic screening
services, prevention education and assistance in the development of
individual health promotion goals.
Home Visits: Provide Health Depot services for people who are not able
to access the community clinics and provide in-home assessments of
health needs.
Join Nelson Volunteer Coalition: A faith-based community organization
which brings people in need into contact with people who can help..

Latino Health Services: We offer special services for Spanish-speaking
residents including interpretation, Health Depots, health education
programs, assistance with paperwork and referrals to other area
resources.
Wellness Passport: A benefit program for low-income people without
health insurance.
Health Education and Support Groups: A series or ongoing support
groups including such subjects as diabetes, weight/nutrition management,
medication management.
The Corporate Wellness Initiative: Introduced In 2005, RHOP introduced
a program that offers area businesses the opportunity to provide health
care services to their employees at their place of business.
Our Mobile Medical Clinic provides on-site, company-sponsored
screenings and physical exams as well as screenings at community
events throughout the year.
Through Virginia’s Family Access to Medical Insurance Security
(FAMIS) initiative, RHOP helps families gain access to Medicaid and other
financial assistance programs.
School Based Health Care: RHOP provides full-time school nursing
services in every Nelson County Public School.
Through the Medication Assistance Program, we are able to make
available to needy patients medications which, otherwise they would not
be able to afford.
